2025

Perfect Vision Laser Eye Hospital – Dr. Virendra Laser Phaco Surgery Centre, Jaipur, offers Johnson & Johnson Vision's ELITA SiLK femtosecond laser platform.
Designed to reduce dependence on spectacles and contact lenses, SiLK combines a small-incision approach with precise laser technology. Treatment begins with a detailed examination to determine whether it is appropriate for your eyes.
1. What is SiLK?
SiLK stands for Smooth Incision Lenticular Keratomileusis. It is a minimally invasive procedure for correcting myopia—short-sightedness—with or without astigmatism, commonly called cylindrical power.
The ELITA laser creates a precisely shaped piece of tissue, called a lenticule, within the cornea—the transparent front surface of the eye. Your surgeon removes this tissue through a small incision, changing the cornea’s shape to help light focus more accurately on the retina.
The procedure does not require a LASIK-style corneal flap.
2. What makes ELITA technology distinctive?
Biconvex lenticule design
SiLK creates a lenticule with curved surfaces on both sides. This design supports matching of the remaining corneal surfaces after the tissue is removed. The lenticule is formed from your own corneal tissue. It is not an artificial lens or implant.
Closely spaced laser spots
ELITA places laser spots close together to create a continuous tissue cut. This approach is designed to minimise tissue bridges and support smooth lenticule separation and removal.
Low-energy laser pulses
The platform combines low pulse energy with tightly focused laser delivery to create the planned treatment surfaces precisely.
Small-incision treatment
The lenticule is removed through a small opening without lifting a large corneal flap. This avoids complications specifically associated with flap creation and displacement.
These features support the surgical process. The outcome also depends on appropriate patient selection, treatment planning, surgical technique and healing.
3. How does SiLK compare with LASIK, Femto-LASIK and PRK?
These procedures reshape the cornea using different techniques. Femto-LASIK is also a flap-based procedure; its distinction from conventional LASIK is how the flap is created.
| Feature | ELITA SiLK | Conventional LASIK | Femto-LASIK | PRK |
|---|---|---|---|---|
| Treatment approach | A laser-created lenticule is removed through a small incision. | A flap is created using a microkeratome blade, followed by excimer laser reshaping. | A femtosecond laser creates the flap, followed by excimer laser reshaping. | The surface epithelium is removed, followed by excimer laser reshaping. |
| Corneal flap | No flap. | Flap-based. | Flap-based. | No flap. |
| Technology used | ELITA femtosecond laser. | Microkeratome and excimer laser. | Femtosecond and excimer lasers. | Excimer laser; the surface-removal method varies. |
| Early recovery | Generally quick functional recovery, with individual variation. | Generally quick visual recovery. | Generally quick visual recovery. | Usually slower initial recovery while the surface heals. |
| Early discomfort | Temporary irritation or discomfort may occur. | Temporary irritation or discomfort may occur. | Temporary irritation or discomfort may occur. | Discomfort is usually more noticeable during early surface healing. |
| Main consideration | Requires suitability for lenticule extraction. | Requires suitability for flap creation and tissue removal. | Laser-created flap still requires appropriate corneal suitability. | May suit selected patients where avoiding a flap is preferred; adequate corneal suitability remains essential. |
Your surgeon will consider your prescription, corneal thickness and shape, eye-surface health, occupation and recovery needs before recommending treatment.
4. How does SiLK treatment work?
Assessment and counselling
Your surgeon examines your eyes, checks prescription stability and evaluates your cornea. You discuss expected outcomes, alternatives and potential risks.
Preparation
Anaesthetic eye drops numb the eye. The team explains what you may feel and how to follow instructions during treatment.
Positioning
Your eye is positioned and stabilised for laser delivery. The surgeon checks alignment before proceeding.
Lenticule creation
The ELITA femtosecond laser creates the planned lenticule within the cornea. The laser stage forms only one part of the complete procedure.
Lenticule removal
Your surgeon carefully separates and removes the lenticule through the small incision. This changes the corneal shape to provide the intended correction.
Recovery and review
You receive instructions for prescribed drops, follow-up visits and returning to daily activities. Recovery is monitored according to your individual needs.
5. Benefits for patients
Reduced dependence on spectacles and contact lenses
SiLK aims to improve unaided distance vision, making everyday activities more convenient. Some patients may still need spectacles for particular tasks.
No LASIK-style corneal flap
The small-incision approach avoids flap-specific complications. This may be relevant to patients with active lifestyles, although sports and exercise must resume only as advised.
Generally quick functional recovery
Many patients notice useful visual improvement early after treatment. Comfortable, stable vision may take longer, and recovery varies between individuals.
Treatment based on your eye measurements
The planned correction reflects your prescription and corneal measurements. Your surgeon determines whether treatment can be performed within suitable limits.
Greater convenience after recovery
Reduced reliance on vision aids can simplify work, travel and exercise without the daily routine of handling contact lenses or wearing spectacles.
6. Who may be suitable?
- Myopia, with or without astigmatism, within the applicable treatment range.
- A stable spectacle prescription.
- Suitable corneal thickness and shape.
- Healthy eyes without conditions that make treatment inappropriate.
- Realistic expectations about recovery and outcomes.
Eligibility requires a complete examination. Age and spectacle power alone are insufficient to determine suitability.
7. Diagnostic tests and evaluation
- Vision testing and measurement of spectacle power.
- Corneal topography or tomography to assess corneal shape.
- Corneal thickness measurement.
- Tear-film and ocular-surface assessment.
- Slit-lamp examination.
- Eye-pressure measurement and retinal evaluation as indicated.
These findings help your surgeon assess safety and recommend the most appropriate treatment.
8. Who may need to postpone treatment or consider another option?
An unstable prescription, suspicious corneal findings, insufficient corneal thickness, significant untreated dry eye or active eye infection may affect suitability.
Pregnancy, breastfeeding, certain medical conditions and some medications may also influence treatment timing. Share your medical history and current medicines during the consultation.
9. Risks and limitations
SiLK is a surgical procedure and cannot guarantee a particular visual outcome.
Possible concerns include dryness, glare, halos, fluctuating vision, residual prescription and problems with healing. Less common complications can include infection, inflammation or corneal weakening.
Some patients may require spectacles or further treatment. Your surgeon will discuss the risks relevant to your eyes before you decide.
10. Preparation before treatment
- Bring previous prescriptions, examination reports and relevant medical records.
- Stop contact-lens wear for the period advised by your clinic.
- Inform the team about medicines, allergies and existing medical conditions.
- Follow instructions about eye makeup and products used around the eyes.
- Arrange transport home after the procedure.
11. Recovery, aftercare and follow-up
- Use prescribed eye drops exactly as directed and attend scheduled reviews.
- Avoid rubbing your eyes. Follow your surgeon’s instructions about water exposure, makeup, swimming, contact sports and dusty environments.
- Return to work and driving should depend on your vision and clinical assessment. Early improvement does not mean healing is complete.
- Contact the centre promptly if you develop increasing pain, marked redness or a sudden reduction in vision.

“#SMILEforeyes First of all very grateful to Dr. Virendra Agarwal and his team for being so supportive and helpful at the critical stage of my life when my hand was fractured in an accident, it was quite arduous to perform surgery but still it was done very well. Opted the SMILE surgery (bladeless, painless, flapless, conducted using top notch technology) and within a month, I am back to the normal routine which is quite impressive.
