CORNEA CARE
Descemet's Membrane Endothelial Keratoplasty – DMEK
DMEK selectively replaces Descemet's membrane and the endothelial pump-cell layer without donor stroma. It is used for suitable endothelial disease when the front cornea is otherwise healthy. Because the graft is extremely thin, surgery and early positioning require precise technique and close follow-up.
DMEK most closely replaces the diseased anatomical layers in endothelial failure. With no donor stroma added, it can offer strong visual potential and a low rejection risk in suitable eyes. The technique is not automatically appropriate for every patient; significant anterior scarring, complex internal anatomy or previous surgery can make another approach safer.
The transparent graft naturally rolls with the endothelial cells facing outward. During surgery it must be correctly oriented, unfolded and centred without excessive handling. An air or gas bubble then supports attachment. Vision can remain hazy while swelling clears, and a partially detached graft may need rebubbling. Follow-up also checks pressure and steroid response.

1. Who may be suitable for DMEK?
Fuchs endothelial corneal dystrophy causing swelling or reduced vision
Selected cases of endothelial failure after cataract or other eye surgery
Eyes with a relatively clear anterior cornea and anatomy suitable for graft unfolding
2. The DMEK procedure
Pre-operative assessment
Corneal clarity, thickness, eye pressure, lens status and previous surgery are reviewed before planning.
Replace diseased cells
The surgeon removes the faulty inner membrane, inserts the rolled donor graft and carefully unfolds it.
Bubble attachment
Air or gas holds the tissue against the back of the cornea while the cells begin restoring fluid balance.
3. Potential benefits and practical limits
Anatomical replacement
Only the layers responsible for endothelial pumping are transplanted.
Visual potential
Suitable eyes may achieve good optical quality because no donor stromal layer is added.
Technically demanding
Very thin tissue can detach or be difficult to position, and some eyes are better served by DSEK.
4. Aftercare and risks
- Face-up positioning may be advised and air travel must be avoided while a gas bubble remains
- Rebubbling, rejection, graft failure, pressure rise, infection or retinal complications can occur
- Prescribed steroid drops and regular checks help protect graft function
