CORNEA CARE

Deep Anterior Lamellar Keratoplasty – DALK

DALK replaces the front and middle corneal layers while retaining the patient's healthy Descemet's membrane and endothelium. It is designed for disease affecting the corneal stroma but sparing the inner pump-cell layer. Careful assessment confirms whether this layer-preserving approach is possible.

DALK is particularly relevant when the main problem lies in the corneal stroma and the endothelium still works normally. Preserving the patient's own inner cells removes the risk of endothelial graft rejection and may support longer graft survival in suitable eyes. It does not, however, remove the need for careful post-operative monitoring.

The surgeon separates the affected stroma close to Descemet's membrane, a delicate inner layer. The depth and pattern of scarring can make this separation more difficult, so full-thickness conversion is sometimes the safer decision during surgery. After healing, residual astigmatism may still require spectacles, a rigid or scleral contact lens, or later refractive discussion.

1. Who may be considered for DALK?

Keratoconus with significant scarring or vision that cannot be rehabilitated adequately

Deep anterior scars or stromal dystrophies with a functioning endothelium

Eyes where preserving healthy endothelial cells offers a meaningful advantage

2. The DALK pathway

  1. Layer assessment

Slit-lamp examination, tomography, thickness and endothelial testing establish the depth of disease.

  2. Anterior tissue replacement

The surgeon separates diseased stroma from the inner membrane and secures donor anterior tissue with sutures.

  3. Recovery and refraction

Drops and reviews support healing; changing astigmatism is monitored before final spectacles or contact lenses are prescribed.

3. Potential advantages and limits

Endothelium is preserved

Keeping the patient's own pump cells avoids endothelial rejection, although other forms of graft rejection remain possible.

Closed-eye surgery

The inner corneal barrier remains intact when the operation proceeds as planned.

Conversion may be necessary

If the inner membrane cannot be safely preserved, the surgeon may need to convert to full-thickness PK.

4. Risks and aftercare

  • Possible issues include interface haze, perforation, infection, rejection, raised pressure and significant astigmatism
  • Visual recovery is gradual because sutures and corneal shape change over time
  • New redness, pain, light sensitivity or reduced vision requires prompt examination

Every eye is different. The right procedure is the one that fits your cornea, your lens, your retina and the way you use your vision every day — not the one that is most advertised. Technology supports the decision; it never replaces the clinical evaluation.
