CORNEA CARE
Corneal Infections
A corneal infection, or microbial keratitis, is urgent and can cause an ulcer, scarring and loss of vision. Bacteria, fungi, viruses or parasites may be responsible. Early assessment matters because treatment differs by organism and severity.
Microbial keratitis can progress quickly because organisms damage transparent corneal tissue and trigger inflammation. Contact-lens wear is an important risk factor, but infection can also follow plant or soil injury, surface disease, previous surgery or reduced immunity. The appearance alone does not always reveal whether bacteria, fungi, a virus or Acanthamoeba is responsible.
Treatment often begins before laboratory results are available. Large, central, deep or unusual ulcers may be scraped so that microscopy and culture can guide medication. Drops may be required hourly at first, including overnight in severe cases. Steroid drops are introduced only when the treating ophthalmologist considers them safe, because inappropriate use can worsen some infections.

1. Warning signs requiring prompt care
Increasing eye pain, redness, watering or strong sensitivity to light
Sudden blurred vision, discharge or difficulty keeping the eye open
A white or grey spot on the cornea, particularly in a contact-lens wearer or after injury
2. How infection is assessed and treated
Urgent examination
The ophthalmologist checks ulcer size, depth, inflammation and any risk of thinning or spread inside the eye.
Microbiology when needed
A corneal scraping may be taken for microscopy and culture before targeted treatment is refined.
Intensive medication
Frequent antimicrobial drops are often started immediately and adjusted according to response and laboratory findings.
3. Common risk factors
Contact lenses
Sleeping in lenses, water exposure, poor hygiene or using expired solution increases infection risk.
Trauma and surface disease
Plant or soil injury, dry eye, eyelid disease or reduced corneal sensation can weaken protection.
Steroid misuse
Unsupervised steroid drops can worsen or mask infection and should not be started without specialist advice.
4. What patients should do
- Stop contact-lens wear and bring the lenses, case and solutions to the appointment if available
- Use drops exactly at the prescribed frequency and attend every requested review
- Seek immediate help if pain, redness or vision worsens; advanced disease may require hospital care or surgery
