STABILISING A PROGRESSIVE CORNEA
Corneal Cross-Linking for keratoconus and ectasia
Cross-linking combines riboflavin with controlled ultraviolet light to strengthen corneal collagen. Its primary aim is to reduce the risk of further progression—not to guarantee clearer unaided vision.
1. What establishes the indication?
Documented or strongly suspected progression
Corneal thickness suitable for the selected protocol
Absence of active infection and review of scarring or other disease
2. The cross-linking process
Baseline mapping
Topography/tomography, pachymetry and vision establish the pre-treatment record.
Riboflavin and ultraviolet exposure
The cornea is prepared according to the chosen protocol, saturated with riboflavin and exposed to controlled ultraviolet light.
Surface healing and monitoring
A bandage lens and medication support healing; serial scans later assess stability.
3. What cross-linking can and cannot do
Stability goal
The aim is to slow or stop further corneal weakening.
Vision rehabilitation is separate
Glasses or specialised contact lenses may still be required after stabilisation.
Timing matters
Treating documented progression before advanced scarring may preserve more future optical options.
4. Recovery and risks
- Pain, watering and light sensitivity are expected early with surface-removal protocols
- Infection, haze, delayed healing or loss of clarity can occur
- Progression can still occur, so follow-up scans remain necessary
